Lync 2013 - EndEstablish fails because target machine actively refused it

I am migrating a Lync 2010 server/application to Lync 2013.  When my application attempts to call EndEstablish for an application endpoint, I get the following error:

Error [Microsoft.Rtc.Signaling.ConnectionFailureException:Unable to establish a connection. ---> System.Net.Sockets.SocketException (0x80004005): No connection could be made because the target machine actively refused it 10.10.7.1:5061

The target machine is my Active Directory server.  Windows Firewall is disabled on the AD server, the Lync 2013 server, and on my application server.

Any ideas on why this is happening would be greatly appreciated.
